Effects of ivabradine in patients with stable angina receiving beta-blockers according to baseline heart rate: an analysis of the ASSOCIATE study.

Achtergrond

Bij patiënten met stabiele coronaire hartziekte (CAD) kan een verhoging van de hartslag tijdens dagelijkse activiteiten verband houden met ischemie [1,2]. Een hartslag in rust van 60 slagen per minuut wordt vaak aanbevolen als een doel in CAD patiënten [3]. De If remmer ivabradine vermindert de hartfrequentie met anti-ischemische en anti-angineuze effectiviteit bij patiënten met stabiele angina pectoris versus placebo en versus bètablokkers en calciumantagonisten [4-7]. Uit de ASSOCIATE (evaluation of the Antianginal efficacy and Safety of the aSsociation Of the If Current Inhibitor ivAbradine with a beTa-blockEr ) studie is gebleken dat ivabradine veilig gecombineerd kan worden met een bètablokker bij patiënten met chronische stabiele angina pectoris, wat extra anti -ischemische effectiviteit biedt [8]. Het doel van deze post hoc analyse van de ASSOCIATE studie was om na te gaan of de anti-angineuze en anti-ischemische effectiviteit en veiligheid van ivabradine bij patiënten met een relatief lage baseline hartslag in rust vergelijkbaar is met die bij patiënten met een hogere hartslag. Patiënten met chronische stabiele angina pectoris werden gerandomiseerd naar behandeling met ivabradine (5 tot 7.5 mg bid) of placebo gedurende 4 maanden, naast 50 mg atenolol od. Het effect van de behandeling op inspanningstolerantie testparameters werd geanalyseerd in twee groepen volgens baseline hartslag (> 65 bpm vs ≤ 65 bpm).

Belangrijkste resultaten

  • Behandeling met ivabradine was geassocieerd met daling van hartslag in rust in beide groepen met placebo-gecorrigeerde verlaging van -9,1 (95% BI -11,0 tot -7,3;> 65 bpm-groep) en -5,9 (95% BI -7,5 tot -4,3; ≤ 65 bpm groep) (beide p <0,001 versus placebo).
  • Na 2 maanden verlaagde behandeling met ivabradine de gemiddelde hartslag in rust tot 63,0 bpm in de> 65 bpm groep en 57,6 bpm in de ≤ 65 bpm-groep en na 4 maanden tot 60,3 bpm in de> 65 bpm-groep en 56,8 bpm in de ≤ 65 bpm groep (fig. 1).
  • Ivabradine verminderde de hartslag in alle stadia van inspanning. De behandeling met ivabradine verlengde de totale trainingsduur in beide hartslag groepen gedurende 4 maanden. Er waren ook toenames in de tijd tot beperkende angina, tijd tot begin van angina en tijd tot 1 mm ST-segment daling (Fig. 2).
  • Er waren geen verschillen tussen de twee hartslag groepen wat betreft veiligheid of bijwerkingen

Conclusie

Ivabradine gaf significante verbeteringen in angina pectoris en in de inspanningscapaciteit in vergelijking met placebo bij patiënten met stabiele angina pectoris behandeld met ß-blokker therapie ongeacht of hun rust hartslag boven of onder de 65 slagen per minuut was.

Abstract

Background:
Any increase in heart rate (HR) during daily activities above the ischemic threshold may trigger myocardial ischemia. HR reduction with the I(f) inhibitor ivabradine has been demonstrated to confer anti-ischemic and antianginal efficacy in patients with stable angina pectoris. This analysis of the ASSOCIATE trial assessed whether the anti-ischemic efficacy and safety of ivabradine were comparable in patients with baseline resting HRs above and below the median.

Methods:
Patients with chronic stable angina pectoris were randomized to treatment with ivabradine (5 to 7.5mg bid) or placebo for 4months, in addition to atenolol 50mg od. The effect of treatment on exercise tolerance test parameters was analyzed in two groups according to baseline HR: >65bpm (n=418) versus ≤65bpm (n=436) (above and below the median, respectively).

Results:
Ivabradine reduced resting HR in both groups with placebo-corrected reductions of -9.1 (95% CI -11.0 to -7.3; >65bpm group) and -5.9 (95% CI -7.5 to -4.3; ≤65bpm group) (both P<0.001 versus placebo). Ivabradine reduced heart rate at all stages of exercise (all P<0.001). Improvements in exercise capacity (total exercise duration, time to limiting angina, angina onset, and 1-mm ST segment depression, all P<0.05) were recorded in both HR groups. There were no differences between the two groups in terms of safety.

Conclusions:
Ivabradine resulted in significant improvements in exercise capacity relative to placebo in patients with stable angina pectoris receiving beta-blocker therapy whether their resting HR was above or below 65bpm.
